The incidence rate of breast cancer in women increased every year in our country, specially, the onset age became younger than before. Individual prevention is very important to decrease breast cancer. Raf/MEK/ERK is the main signaling pathway to activate cancer genes, which palys important role in the development of breast cancer. Studies in vitro suggested that Raf/MEK/ERK signaling pathway could induce the cell differentiation, loss of apoptosis, abnormal proliferation, and the development of cancer. mRNA levels could be disturbed by the methylation status of DNA or miRNA levels, and further impact proteins. Since no epidemiological study about the relationship between DNA methylation, related miRNAs in Raf/MEK/ERK signaling pathway and the risk of breast cancer has been reported. In this research, we will focus on the following aspects: the association between DNA methylation and environmental factors, and their interaction with the risk of breast cancer; the association between miRNA methylation, miRNA levels and the risk of breast cancer; moreover, we will systematically analyze the interaction of DNA methylation, miRNA methylation status and expression levels, and the environmental factors in the development of breast cancer. All of these will supply the basis for the individual prevention of breast cancer.
我国妇女乳腺癌的发病率逐年增加,且呈发病年轻化的趋势,有效的个体化预防,是减少乳腺癌发生的关键。Raf/MEK/ERK作为癌基因活化的重要通路,在乳腺癌的发生中起了重要作用。体外研究表明:由Raf激活的Raf/MEK/ERK通路可导致细胞分化、凋亡功能的丧失,细胞异常增殖乃至肿瘤发生。mRNA水平受到DNA的甲基化、miRNA的干扰都会导致蛋白表达水平的差异。基于该通路基因甲基化水平、相关miRNAs与乳腺癌关系的人群流行病学研究尚处空白。本项目采用病例对照研究,探讨Raf/MEK/ERK通路基因甲基化水平与环境因素暴露的关系、及其交互作用与乳腺癌发病的关系;靶向相关的miRNAs基因启动子区甲基化及其表达水平与乳腺癌发生的关系;系统阐述Raf/MEK/ERK通路基因甲基化、相关miRNAs的甲基化及其表达水平、环境因素交互作用与乳腺癌发病的关系,为乳腺癌的个体化预防提供具体的指标和依据。
研究背景:Raf/MEK/ERK作为癌基因活化的重要通路,在乳腺癌的发生中起了重要作用,而基于该通路基因甲基化水平、相关miRNAs与乳腺癌关系的人群流行病学研究尚处空白。.研究内容:采用病例-对照研究方法,RT-PCR检测血浆中通路相关microRNA的水平,应用ROC曲线评价microRNAs的诊断学价值;高分辨率溶解曲线(HRM)方法检测外周血DNA中通路相关基因甲基化水平,应用多因素回归统计分析DNA甲基化及其环境因素交互作用与乳腺癌发生的关系。采用单纯病例研究,检测乳腺癌肿瘤组织DNA中通路相关DNA的甲基化状态,并分析其与环境因素暴露、临床病例特征的关系。.研究结果:1)经分析,外周循环血浆miR-497、miR-195、miR-125b、miR-221、miR-222在病例组和对照组中的差别有统计学意义,且均在乳腺癌病例中高表达;应用mir-497、mir-125b对乳腺癌进行联合诊断,ROC曲线下面积(AUC)为0.732(95%CI: 0.675-0.790);应用mir-497、mir-125b、mir-221和mir-222对乳腺癌进行联合诊断:ROC曲线下面积(AUC)为0.791(95%CI: 0.704-0.879);2)未发现外周血白细胞DNA中MKP-1的甲基化水平与乳腺癌的发生有显著关联,但外周血DNA的MKP-1甲基化、肿瘤组织MKP-1的甲基化均与ER/PR阴性有显著的统计学关联;ER与PR阴性的患者人群中,过量的水果摄入(>1000g/week)与降低的MKP-1甲基化水平显著相关;在ER与PR阳性的患者人群中,月经不规律与MKP-1的甲基化水平增加显著相关;3)未发现外周血白细胞DNA中RIN1的甲基化水平与乳腺癌的发生、及临床病例特征有显著关联; 4)肿瘤组织DAB2和MAP3K1基因甲基化在临床病理特征分布上均不存在差异,但与乳腺疾病等环境因素显著相关。.科学意义:首次研究表明外周循环血浆miR-497、miR-195、miR-125b、miR-221、miR-222在乳腺癌中的诊断学价值,该通路microRNAs可联合作为乳腺癌潜在的辅助诊断指标;MKP-1在白细胞DNA中及肿瘤组织DNA中的甲基化水平与ER/PR阴性状态显著相关,食用豆浆及月经不规律对组织MKP-1甲基化有显著影响,对乳腺癌患者预后治疗有重要指导意义
