古方青娥丸介导Wnt-DDK-1信号转导通路调控绝经后骨代谢的机制研究

With the advent of aging society, osteoporosis has become an increasingly serious public healtth preoblem in the current world. The high incidence of post-menopause women suffer from osteoporsis,so searching for safe and effective durg is difficulty and hot in the domain of medicine.Our previous study confirmed that the effect of Qing E pill,a traditional ancient formula,in treating postmenopausal osteoporosis was notable and this therapeutic effect is associated with it can regulate the levels of plasma Wnt/β-catenin - OPG singaling pathway. Recently,some scholars report that the Wnt-DDK1 singaling pathway plays an important role in Bone loss disease. Our pre-tests showed that this traditional ancient formula could down-regulate the expression of DDK-1 in callus of ovariectomized rats with osterporosis fracture and promoting fracture healing. Therefore,we infer that regulating Wnt-DDK-1 signaling pathway perphaps is one of the most important mechanisms of Qinge pill to treat postmenopausal osteoporosis.Based on this point, in the present study,flow cytometry, Western blot, ELISA techniques and other experimental methods will be used for dynamic observing DKK-1 concentration or activity changes of patients with postmenopausal osteoporosis before and after Qing'e Pill treatment; and implement appropriate animal experiments, in vitro cytological studies for identifying Qinge pills action target and providing theoretical basis for actual application of the traditional ancient formula.

随着人口老龄化,骨质疏松症已成为严峻的公共卫生问题。绝经后妇女骨质疏松症发生率高,因此,研究治疗本病的安全、有效药物是医学上的难点及热点。本课题组前期发现古方青娥丸能提高骨质疏松症患者骨密度,其机制与其能调控Wnt/β-Catenin信号通路,上调骨保护蛋白表达有关。新近有学者发现Wnt信号通路抑制因子DKK-1与骨量丢失疾病关系最为密切。近期预实验显示:青娥丸可下调骨质疏松模鼠骨折骨痂DDK-1的表达,促进骨愈合。由此我们推测青娥丸治疗绝经后骨质疏松症疗效机理之一,可能与其调控Wnt-DDK-1信号通路有关。本项目拟采用流式细胞术、免疫印迹、酶联免疫吸附试验等技术,动态观察青娥丸治疗绝经后骨质疏松症患者过程中,DKK-1浓度或活性变化;并实施相应的动物实验,体外细胞学研究,对比分析青娥丸是否可通过调控该信号通路治疗绝经后骨质疏松症,以期明确该方的作用靶点,为其推广应用提供理论依据。

项目摘要

随着人口老龄化,骨质疏松症的发病率愈来愈高,已成为严峻的公共卫生及社会问题。绝经后妇女骨质疏松症发生率高达60%~70%,因此骨质疏松症研究的重点在中老年妇女。骨质疏松症患者除疼痛、骨骼变形外,其直接不良后果是骨质疏松性骨折。与骨质疏松性骨折相关的死亡率已超过乳腺癌、宫颈癌和子宫体癌死亡率的总和,即使采取手术治疗,术后仍需常规给予抗骨质疏松药物,以防止再骨折的发生。 .应用中医药开展抗骨质疏松治疗已使我国骨质疏松症研究在国际学术界独树一帜。根据中医传统理论, 结合现代医学对本病发病机理的认识, 对有代表性的方药、治则进行综合的研究和评价, 以筛选出安全和有效的中药制剂和治则, 是目前防治骨质疏松的当务之急, 也是今后深入研究的方向。 .为此,本研究在国家自然科学基金面上项目(No.81473492)的资助下,重点研究了骨质疏松相关细胞信号转导通路,并动态观察了古方青娥丸抗骨质疏松的作用环节。首先,我们在中医基础理论研究中,首次提出并用现代检测手段探讨证实原发性骨质疏松症的核心病机属于中医“本痿标痹”,并在“肾主骨”理论指导下,探讨了“齿、发、骨”的生理病理变化与人体之肾气密切相关,为中医药的治疗提供坚实的理论基础。其次,我们系列完善的信号通路基础研究中,发现骨质疏松症Wnt/β-Catenin的细胞传导信号表达明显低于正常,Wnt信号通路抑制因子DKK-1及骨硬化蛋白(Sclerostin, SOST)的活性明显增高,并与骨密度,骨微结构、骨代谢标志物及相关细胞因子有明显的相关性,为中医药的治疗研究提供了理论靶点。本研究运用补肾强骨中药青娥丸治疗骨质疏松症,具有减轻或缓解患者腰背疼痛症状之功,其机理可能是通过影响上述信号通路的不同靶点,如DKK-1、SOST等,从而抑制Wnt/β-Catenin细胞转到,最后影响下游的骨转换因子的表达发挥作用。.; ; ; ;
