Trauma has become the third leading cause of death in China. The mortality of traumatic brain injury (TBI) ranks first in all kinds of trauma. Medical imaging examination is recognized as the basis of clinical treatment options. The existence of reversible tissue around the primary injury (i.e. traumatic penumbra, TP) is currently the focus of clinical attention. Yet there is lack of imaging diagnostic criteria for TP as well as systematic study. Our prior experimental animal study on TP of focal cerebral trauma combined electronic microscopy showed optimistic result that there is mismatch between structural magnetic resonance imaging and perfusion imaging, which outlined the penumbra. This project aims to use multimodal imaging to evaluate the hemodynamic changes, analyse the edema type and detect the metabolites of TP. The final aim is to reveal the relationship between TP and reversible tissue damage which may lead to secondary brain injury, and to establish imaging diagnostic criteria of TP, and then achieve the goal of early diagnosis and early warning for TBI patient.
创伤已成为我国第三位的死因,创伤性脑损伤(traumatic brain injury,TBI)的死亡率居各类创伤的第一位,脑创伤发生后影像学检查是选择治疗方案的基础。在TBI原发损伤周围的可逆性组织(称做创伤性半暗带)是临床治疗关注的焦点,然而对半暗带的研究缺乏影像学诊断标准和系统性研究。本项目在前期实验动物的局灶性脑创伤半暗带的研究中,结合电镜观察,发现磁共振成像结构像和灌注成像之间存在失配,即半暗带的界定。本项目拟采用多模态影像学新技术和新方法,评价半暗带的血液动力学变化,应用磁共振弥散技术区分其中水肿类型,根据MRS代谢产物测定、分析结果评估神经元损伤,进一步揭示创伤性半暗带与继发性损伤的因果关系,以期建立医学影像早期诊断局灶性TBI半暗带的标准和诊断方案,有望在脑创伤可逆性组织的早期诊断、继发性损伤的预警方面取得成果。
在创伤性脑损伤时,创伤灶周围组织随时间进展而发生着微观变化,如不加干预便可能发生继发性损伤,因此,发现神经细胞继发损伤的时间进程对选择治疗时机至关重要。本项目应用磁共振影像技术对创伤性半暗带研究,将脑创伤的影像诊断向判断可逆性组织存在与否的方面引导。在局灶性创伤性脑损伤的动物模型的基础上,应用磁共振的弥散张量成像(Diffusion Tensor Imaging, DTI)技术,对创伤灶周围的水肿演变规律、纤维束受累情况进行了研究,发现了不同时间点创伤灶周围区ADC值、FA值的变化规律,特别是ADC值和FA值的恢复的时间拐点,得出了创伤灶周围的细胞毒性水肿不断进展的结果,和轴索破坏、髓鞘损伤,纤维排列致密性降低的结果,同时,应用磁共振波谱(Magnetic Resonance Spectroscopy, MRS)成像技术对创伤灶周围代谢产物的变化加以分析,发现了代谢产物的变化规律,特别是具有统计学意义的代谢产物变化时间点;并据此得到提示局灶性创伤性脑损伤后3h前可能是治疗可逆性损伤的时间窗,为局灶性创伤性脑损伤的诊断及治疗提供了新的思路。
