广西乡镇卫生院精神卫生服务绩效评价指标体系的构建与实证研究

Severe mental illness has already become a great public health problem and outstanding social issues in China and Guangxi area. Community mental health services is the fundamental way out for severe mental illness control in rural areas of China. In recent years, both the central and local governments are paying more attention to the benefits while increasing investment in mental health services. So, establishing Comprehensive scientific evaluation system is the basic foundation and core way for evaluating the performance of Community mental health services. At present not only the literature about performance evaluation of mental health services is rarely at home, but also the performance assessment indicator system, too. By drawing on the domestic and foreign existing therapy and method about performance evaluation, the project will develop performance assessment indicator system of township hospital mental health service meeting the recent facts of rural community in Guangxi through field investigation and Delphi, and then verifies its reliability and validity. Next, taking the Township hospital in Guangxi as the evaluation unit, the project aims to establish the integrated assessment models about rural community mental health services by factor analysis technique. At last, the project will use the integrated assessment models to evaluate the performance of township hospital mental health service, and analyze the related factors. Based on above content, the project will also give policy advice in order to improve the performance of township hospital mental health service.

重性精神疾病已成为我国和广西重大的公共卫生问题和突出的社会问题。社区精神卫生服务是我国尤其农村地区重性精神疾病防治工作的根本出路。近年来，中央和地方各级政府不断增加精神卫生服务投入同时，亦关注投入是否发挥了降低精神疾病的发生和危害的效益。精神卫生服务绩效评估能有效地促进精神卫生服务质量的改善。建立科学全面的绩效评价指标体系是绩效评价的基础和核心。但国内鲜见精神卫生服务绩效评价文献，社区精神卫生服务评价指标体系研究尤其缺乏。本项目借鉴国内外已有绩效评价的理论和方法，通过现场调查和德尔菲(Delphi)法，研制科学可行的符合广西实际的农村社区精神卫生服务绩效评价指标体系，并检测指标体系的信度和效度；以广西乡镇卫生院为评估单位，采用因子分析方法构建农村社区精神卫生服务综合评估模型，综合评价广西农村社区精神卫生服务绩效及分析影响绩效的主要因素，从而提出改善广西农村社区精神卫生服务绩效的政策建议。

严重精神障碍是重大的公共卫生问题和突出的社会问题。乡镇卫生院承担严重精神障碍管理和相关精神卫生服务工作。因此，开展乡镇卫生院精神卫生服务绩效评价研究对促进农村精神卫生服务质量的改善有重要意义。. 本项目采用“结构-过程-结果”模型，通过文献研究、访谈和德尔菲咨询等方法，构建乡镇卫生院精神卫生服务绩效评价指标体系；采用因子分析方法构建了绩效综合评估模型，评价乡镇卫生院精神卫生服务绩效及其影响因素，从而提出改善乡镇卫生院精神卫生服务绩效的政策建议。.经过两轮Del Phi专家咨询，确定了乡镇卫生院精神卫生服务绩效评价指标体系：3个一级指标、11个二级指标和49个三级指标。指标体系的总体Cronbach’s α系数为0.782；应用因子分析提取了前5个主成分，其累积贡献为75.61%，显示有较好信度和效度。广西乡镇卫生院精神卫生服务存在如下主要问题：精神卫生服务经费投入不足；精神卫生服务管理以兼职人员为主，且数量不足、质量不高；严重精神障碍患者管理服务能力仍有待提高；精防人员工作负荷大、工作满意度和薪酬满意度低、职业成就感缺乏；医务人员和农村居民精神卫生知识知晓率低，对精神障碍患者存在偏见。采用因子分析法构建了乡镇卫生院精神卫生服务绩效综合评估模型：由5个公因子组成，F综合=0.3814 F基本医疗服务+0.2200F投入产出+0.1466F知识态度+ 0.1375F政策支持+ 0.1145F运营保障，并对45所样本乡镇卫生院进行了实证评价。. 本项目提出如下政策建议：（1）建立健全政府领导、部门合作、社会参与的精神卫生工作机制；（2）建立健全以政府投入为主,多渠道的精神卫生工作投入机制；（3）加强乡镇卫生院精神卫生健康管理人员队伍建设；（4）提升乡镇卫生院严重精神障碍患者的管理服务能力，建立健全精神障碍社区康复服务体系；（5）加强精神卫生健康教育，提高农村居民精神健康素养。. 本项目研究成果可为开展精神卫生服务绩效评价提供新的理念和思路；为国内其他地区基层医疗卫生机构开展精神卫生服务绩效评价提供方法学借鉴。
