Arrhythmia is an important risk factor leading to high mortality of heart disease. MiRNA-1/133 can target the encoding protein genes related to ion channels, thereby regulating the channel currents as well as cardiac electrical activity to affect the heart rhythm. Guizhi Gancao Decoction used for arrhythmia treatment with effects of warming Yang and nourishing Qi.Previous studies found that Guizhi Gancao Decoction has the effects of regulating potassium and calcium channel currents, and shortening the duration of arrhythmia induced by aconitine and calcium chloride.However,the effects and mechanisms on regulating gene and protein expressions ion channel regulated by miRNA-1/133 remain unclear.Therefore, this experiment focuses on miRNA-1/133 and relevant genes and proteins of potassium and calcium channel in an ischemic arrhythmia rat model, to explore the effects and mechanismof Guizhi Gancao Decoction..The results of this study will provide theoretical and experimental basis for traditional Chinese Medicine treatment for cardiac arrhythmia.
心律失常是导致心脏疾病高死亡率的重要危险因素,miRNA-1/133可靶向作用于编码离子通道蛋白相关基因,从而调控通道电流调节心脏电活动影响心律。桂枝甘草汤是仲景温阳益气、治疗心悸的经典方。前期研究发现,桂枝甘草汤具有调节钾、钙离子通道电流,明显缩短乌头碱、氯化钙所致心律失常持续时间的作用。但其是否能够通过调控miRNA-1/133调节离子通道基因和蛋白的表达量,进而影响通道电流发挥抗心律失常作用尚不清楚。因此,本实验围绕miRNA-1/133,在缺血性心律失常模型的基础上,探讨桂枝甘草汤对miRNA-1/133及钾、钙通道基因和蛋白的影响,阐明桂枝甘草汤防治缺血性心律失常的作用机制,为中药抗心律失常作用提供理论和实验依据。
急性心肌梗死恢复血流供应过程中可能引起心肌细胞进一步的损伤或坏死,这被称为心肌缺血再灌注损伤。心律失常的发生涉及到受体、离子通道、microRNA等多种因素的异常改变。研究发现,miRNA-1/133在心肌中的表达量和特异性程度均较高,miRNA-1调节的靶基因是KCNJ2,心肌缺血时miRNA-1表达水平升高,对KCNJ2/ Kir2.1/IK1转录后抑制增强,减少蛋白表达,心脏电传导减慢,导致心梗后心律失常的易感性增加,引发缺血性心律失常。缓慢延迟整流钾电流(IKs)的编码基因KCNQ1和KCNE1的表达受转录因子miRNA-133和miRNA-1共同调节,其蛋白表达量是两者相平衡的结果。miRNA-133可靶向抑制KCNH2和KCNE2的表达,从而调控hERG钾通道(IKr)的表达。KCNQ1和KCNH2是引起LQTS两种最重要的基因,该基因及其产物发生突变或功能障碍可导致遗传性LQTS或获得性LQTS,与致死性室性心律失常引起的高危性心源性猝死密切相关。miRNA- 133a过表达小鼠QT间期以及心肌细胞动作电位时程(APD)延长,Ito明显减小。CaV 1.2主要在心室表达,分别由CACNA1C和CACNB1编码,miRNA-1和miRNA-133表达上调可抑制L-型钙通道的表达。从结果看,miRNA-1和miRNA-133通过作用于K+和Ca2+通道调节心肌细胞电生理活动,从而参与心律失常的发生发展过程。本研究结果表明:①高剂量组桂枝甘草汤能够有效稳定缺血再灌注大鼠的心率,减少心律失常的发生率,降低心律失常的评分,证实桂枝甘草汤具有防治大鼠缺血再灌注心律失常的作用。②桂枝甘草汤能够上调心肌组织基因KCND2、基因KCNQ1、基因KCNH2/KCNE2和基因KCNJ2的相对表达量;并上调ICa-L通道基因CACNA1C/CACNB1的相对表达量,但是桂枝甘草汤对KCNE1作用不显著,无统计学差异。③通过 Western blot 进一步研究发现桂枝甘草汤增加各组心肌组织钾离子通道蛋白Kv4.2、蛋白Kir 2.1、蛋白KvLQT1和HERG的相对表达量的影响,对L-型钙离子通道蛋白Cav1.2的相对表达量也起到升高作用,通过对离子通道蛋白的上调作用,对抗缺血及再灌注状态下减小的离子通道电流,降低心梗后心律失常易感性起到防治缺血再灌注心律失常的作用。
