The specificity of acupoint effects is the key of acupuncture effect.There are some affecting factors in the studies of the specificity of acupoint effects, different manipulative intervention being one of the important factors, but its resulting difference is still unknown. Based on this, the research is conducted under the guidance of Traditional Chinese Medicine theory, assuming the hypothesis that different acupuncture methods have impacts on the specificity of acupoint effects, and the effects can be found in the gene expression profile. Using acupoints according to the meridian acupuncture therapy which effectively mitigates migraine as the vector for the study, taking the advantages of gene chip technology to high-throughput, massively parallel research, integrating acupuncture and moxibustion, molecular biology, bioinformatics, computer science technology, systematically observing hyperactivity of liver-yang type migraine patients treated by manual acupuncture and electroacupuncture acupuncture and acupoint for the pattern of endogenous pain-causing or pain-easing substances in the related gene expression profile, intend to find the key affecting factors the embodiment the feature genes, searching for the biochemical pathways, to provide reliable experimental basis for the clinical effect of acupuncture and moxibustion, also to lay the foundation for exploring the efficacy evaluation of specific molecular markers, in order to reveal the gene expression level on key influence factors of the embodiment of the molecular mechanism.
经穴效应特异性是针灸取效的关键,在经穴效应特异性研究中存在一些相关影响因素,不同的手法干预是其中重要影响因素之一,其影响产生的差异目前尚不清楚。基于此,本研究在中医理论指导下,密切围绕"不同针刺方法对经穴效应特异性产生一定的影响,其影响效应信息基因表达有征可寻"的研究假说,以循经取穴针刺治疗有效的偏头痛为研究载体,利用基因芯片技术高通量、大规模并行研究的优势,整合针灸学、分子生物学、生物信息学、计算机科学等多学科技术方法,整体、系统观察手针与电针针刺经穴与非穴后,肝阳上亢型偏头痛患者内源性致痛物质或止痛物质相关基因的表达规律,拟发现经穴效应关键影响因素体现的特征基因,寻找经穴效应体现的生化途径,为针灸临床疗效的最大获取提供可靠的实验依据,也为探寻疗效评价的特异性分子指标奠定基础,以期在基因表达层面揭示经穴效应的关键影响因素体现的分子机制。
本研究通过基因芯片技术观察手针与电针不同干预方法对偏头痛患者外周血中的基因表达,同时观察经穴与非经非穴比较的基因表达谱。揭示不同针刺干预措施治疗偏头痛的相关基因表达的生化途径,为经穴效应特异性及关键影响因素的机制研究提供科学依据,同时也为提高针刺治疗偏头痛的临床疗效提供科学的实验依据。研究结果显示,电针经穴组治疗后筛选出5个差异基因,电针非穴组治疗前后筛选出11个差异基因,手针经穴组治疗前后筛选出41差异基因,手针非穴组治疗前后筛选出118个差异基因, 由此可见,治疗前后各组均有改变,电针治疗前后差异性较小,可能作用途径较为单一,传统针刺治疗前后差异性较大,可能作用途径较为广泛,经穴与非经非穴比较,经穴组的基因表达的作用机制较为集中,非穴组的作用机制较为分散、模糊,这从一定程度上证明了“经穴的效应信息基因表达有征可寻”,同时也证明了“不同针刺方法对经穴效应特异性产生一定的影响,其影响效应信息基因表达有征可寻。”
