Sjogren's syndrome (pSS) is a common kind of rheumatism and 2/3 of the patients will cause multi-system involvemen. The risk of the disease associated with non Hodgkin's lymphoma is 20 times of the ordinary people. Modern medicine is lack of effective treatment for it. TCM holds that "Moisture deficiency, dry poison, blood stasis" are the basic pathogenesis of pSS. Accordingly, Jiedu Tongluo shengjin prescription is the special prescription for treating pSS in clinic. In the early clinical research and animal experiments, the research group found the prescription can improve patients’ mouth and eyes dry symptoms and the immunological indexes. Regulatory T cells and PD-1/PD-L1 pathway are two important mechanisms to maintain peripheral tolerance. Some independent studies have found that: PD-1/PD-L1 pathway and Treg cells are both involved in the onset of pSS. However, the interaction of PD-1/PD-L1 pathway and Treg cells with the effect of prescriptions in the pathogenesis of pSS have not been studied so far. Therefore, on the basis of preliminary studies, this project intends to explore the changes of PD-1/PD-L1 pathway on Treg cells in pSS model mice with the effect of Jiedu Tongluo Shengjin prescription, then seek the targets of this prescription, and ultimately reveal the mechanisms of Jiedu Tongluo Shengjin prescription treating pSS from the modern molecular and cellular immunology.
干燥综合征(pSS)是一种常见的风湿病,2/3的患者有多系统的受累。该病并发非霍奇金淋巴瘤的风险是普通人的20倍,现代医学缺乏有效的控制病情及进展的方法。中医学认为“滋养润泽不足、燥邪盛实成毒、瘀血干血化燥” 是pSS的基本病机,据此病机而立的解毒通络生津方是我科治疗pSS的专病专方。课题组在前期临床及动物研究中发现该方可改善口眼干燥症状及免疫性指标。调节性T细胞(Treg)与PD-1/PD-L1 通路是维持外周免疫耐受的重要机制。多项独立的研究发现pSS的发病与PD-1/PD-L1通路、Treg 细胞相关,但关于中药方剂如何在pSS中通过PD-1/PD-L1通路影响Treg细胞功能未见深入研究。因此,本课题拟在前期研究工作基础上,探讨解毒通络生津方对pSS模型鼠Treg细胞及PD-1/PD-L1通路的影响,进而探寻该方的作用靶点,最终从分子细胞免疫学的角度揭示该方治疗pSS的的作用机制。
原发性干燥综合征(primary Sjögren's Syndrome,pSS)是一种常见的慢性自身免疫性疾病,发病率仅次于类风湿关节炎。约2/3的患者有内脏器官受累而出现多系统损害的症状,其血清中存在多种自身抗体和高免疫球蛋白,同时pSS发生非何杰金氏淋巴瘤的危险是正常人的20倍。现代医学对pSS的治疗以改善患者局部干燥症状为主,目前无确切作用依据的全身性治疗药物,中医药弥补了西药治疗pSS的缺陷,很多临床研究表明中医药治疗pSS患者有不错的疗效。pSS属中医学燥痹范畴,解毒通络生津方是根据燥痹“滋养润泽不足、 燥邪盛实成毒、 瘀血干血化燥”的三大病机,创制的专病专方。本项目通过体内实验、体外实验、阻断实验三个角度验证PD-1/PD-L1通路对Treg的影响及解毒通络生津方的干预作用。体内实验研究发现,解毒通络生津方对NOD小鼠的颌下腺有很好的保护作用;免疫组化结果显示:解毒通络生津方能上调NOD小鼠颌下腺炎症部位PD-1、PD-L1、Foxp3的表达缓减其炎症反应;流式细胞检测结果显示:解毒通络生津方能增加NOD小鼠颌下腺CD4+Foxp3+PD-1+Treg细胞频数,增强了Treg细胞对Teff细胞的抑制作用;ELISA实验结果显示:解毒通络生津方能明显减少NOD小鼠血清IgG、IgA、IgM的表达。我们在体外细胞实验中证实了解毒通络生津方能降低总的CD4+T细胞中CD4+CD25-Teff的占比,促进了CD4+CD25+Treg细胞的增殖活化。最后,我们在阻断实验研究中发现PD-1信号通路阻断之后,解毒通络生津方失去了体内实验中的治疗作用,证实了PD-1/PD-L1可能是解毒通络生津方治疗pSS的一个作用靶点。总之,通过本项目的研究,我们证实了解毒通络生津方对pSS模型NOD小鼠的治疗作用,从分子学角度阐明了解毒通络生津方配伍的科学性并为其在临床上的应用提供更多的理论依据。
