Abstract: Recent study found that inflammatory cytokines play an important role in the occurrence and development of CRF, and pathologic mechanism remains uncertain. In the state of CRF, the main expressions of inflammatory cytokines are data increases in IL-1, IL-6 and TNF-a. Considering pathologic material, we can say that inflammatory cytokines belong to "phlegm, dampness, turbidity, toxin" which is a traditional Chinese medicine concept. Lucid yang is not ascending and turbid yin is failing to descend that is the main pathological mechanism of "phlegm, dampness, turbidity, toxin" in the state of CRF and we innovate the treatment method based on the therapy of supplementing qi, activating blood, clearing heat and removing toxin and combine rising the lucidity with dropping the turbidity. From our recent research the ascending Qing and descending Zhuo prescription whose principal herbs are cohosh and rhubarb has more positive effects but its concrete regulation mechanism is unclear. Our research group has confirmed that the ascending Qing and descending Zhuo prescription in treating CRF aims to remove the patients' "phlegm, dampness, turbidity, toxin" to implement its treatment. So we make an assumption: the ascending Qing and descending Zhuo prescription obviously inhibit the expression of inflammatory cytokines. In our study we plan to employ this method in animal experiment during which we'll take duplicate rat model as the subject to confirm our assumption and from the angle of inflammatory cytokine network to reveal the mechanism of the ascending Qing and descending Zhuo prescription in blocking the progress of CRF. This study presents a new way of thinking and becomes a new target in the treatment of CRF.
近年研究发现炎症因子对CRF的发生和发展起重要作用,但机制不详。CRF状态下的炎症因子表达主要为IL-1、IL一6和TNF-a等数据升高。从其病理物质来看,炎症因子可归属为中医学"痰、湿、浊、毒"等范畴。我们认为清阳不升,浊阴不降是导致CRF状态下"痰、湿、浊、毒"的主要病理机制,并在以往益气活血清热解毒法基础上创新治法,升清与降浊联合应用。我们的研究显示,以升麻配大黄为主药组成的升清降浊方治疗CRF疗效更为确切,但具体的调控机制尚不明了。本课题组已经证实,升清降浊方对CRF治疗是着眼于对患者"痰、湿、浊、毒"的清除来实现它的治疗效果。因此我们推断,升清降浊方能使CRF发展中的炎症因子的表达明显降低。本研究拟利用该技术进行动物实验,将复制的CRF大鼠模型作为实验对象来证实我们的假说,从炎症因子网络角度揭示升清降浊方阻断CRF进展的作用机制。此研究将为CRF的防治提供新思路和新靶点。
