Our previous studies have shown that :The human body in diesease states will happen sensitive acupoints .The thermal state of acupoints is highly sensitive to thermal moxibustion AI and easy to stimulate the expansion of heat conduction ,heat,gas transmission activities ,focal arrival of acu-esthesia ,so in that way it can improve the curative effect of moxibustion therapy.The thermal state of acupoints is the best acupoint moxibustion,so how fast,accurate,objective and sensitive state is an important part of clinical application of acupoints thermal moxibustion technology.For the moment,there are three methods that frequently used to check the heat sensitive acupoints just like moxibustion method ,temperature threshold method and infrared.In this study ,we choose three diseases which are having advantages of curative effect when use the heat sensitive moxibustion as the subject.The three diseases are the functional dysmenorrhea ,prolapse of lumbar intervertebral disc (acute stage )and KOA(swelling type ).Also ,we make the temperature threshold determination and the infrared thermal imaging technology as the detection method of heat sensitive acupoints .Then we use those two methods to detect the three kinds of disease area of heat sensitive acupoints to judge the different between the test results with the moxibustion group and then to analysis the thermal characteristics of both detection and judging the credibility of the acupoints .In order to find an effective clinical ,objective ,accurate detection of the heat sensitive acupoints and also for the clinical differentiation of sensitive acupoints ,eliminate sensitive moxibustion ,to provide scientific guidance to curative effect of moxibution therapy and then provide a scientific basis to abundant the theory of acupoint sensitization .
课题组以往的研究已表明:人体在疾病状态下,体表腧穴会发生热敏化,热敏态腧穴对艾热悬灸具有高度敏感性,极易激发透热、扩热、传热等经气感传活动,气至病所,从而提高灸疗疗效。热敏态腧穴是灸疗的最佳选穴,如何快速、准确、客观的寻找热敏态腧穴是临床应用热敏灸技术的重要环节。目前,检测热敏化腧穴常用的手段有灸感法、温度阈值法及红外法,本研究以热敏灸有疗效优势的原发性痛经、腰椎间盘突出症(急性期)、膝关节骨性关节炎(肿胀型)3种病症患者为研究对象,以温度阈值测定与红外热断层成像技术为热敏化腧穴的检测手段,在三种疾病热敏化腧穴的高发穴区进行检测,判断腧穴的不同状态,将检测结果分别与灸感法比较,分析判断两者对腧穴的热敏特性的检测的可信性。以期为临床寻找一种高效、客观、准确的热敏化腧穴检测技术,为临床辨敏取穴、消敏施灸、提高灸疗疗效提供科学指导,为丰富腧穴敏化理论提供科学依据。
陈日新团队在临床研究中发现,人体在病理状态下,体表腧穴会发生敏化,敏化腧穴有不同的类型,其中对艾热敏感的称之为热敏化腧穴。目前探查热敏化腧穴的方法主要有灸感法、红外法及温度阈值法三种。其中灸感法是临床应用最广泛的热敏化腧穴探查方法,其优点是客观准确,但是有较费时费力之不足,另外,还受到患者精神状态的限制,目前尚不能满足高效、客观、准确的临床需求。红外发与温度阈值检测法都是客观检测热敏化腧穴的方法,但对于二者的优劣性尚无系统研究。为了对比两种客观检测方法的可信性,本课题以热敏灸有疗效优势的膝关节骨性关节炎(肿胀型)、腰椎间盘突出症(急性期)、原发性痛经3种病症患者作为研究对象,对3种病证热敏化腧穴的高发穴区分别进行灸感法探查、红外法检测和温度阈值法检测,将检测结果分别与灸感法探查结果比较,分析两种检测方法的可信性(敏感性、特异性和准确性),以期为临床寻找高效、客观、准确的热敏化腧穴检测技术,为临床辨敏取穴、消敏施灸、提高灸疗疗效提供科学指导,为丰富腧穴敏化理论提供科学依据。
