欠发达地区农村社区老年健康促进机制-基于健康不平等和社会资本异质性视角的分析

From the perspectives of the health inequality and social capital heterogeneity, the project imports the concept of the social capital heterogeneity(SCH) to the analysis of the elderly health promotion in the less developed rural China community. It also discusses the elements and characters of the SCH, elaborates some issues including the impact mechanism of health inequality and SCH on the elderly health promotion, the diffusion rules of health promotion, the "opinion leader" generating mechanism in the less developed rural China community...With the help of the field survey, case study and experimentation, the project explore the mechanism of the elderly health promotion followed the analysis of the multi-dimensional social capital in the less developed rural China community. With the Hierarchical Linear Model(HLM), the effects of the SCH on the rural elderly health is discussed; With the Mixed Logit Model, the effects of health inequality and the SCH on health promotion is discussed; With the extended Bass Model，the diffusion rules of health promotion among the elderly and the generating mechanism of "opinion leader" in the rural community are discussed. Finally, policy implications are exploited.

从健康不平等和社会资本异质性的视角出发，将社会资本异质性的概念引入欠发达地区农村老年健康促进问题的研究，探讨社会资本异质性的内涵和特征，试图阐明健康不平等和社会资本异质性对欠发达地区农村社区老年健康促进的影响机理，揭示健康促进在农村社区的扩散规律和农村群体"意见领袖"的形成机制等关键问题。.运用田野调查、典型案例和实验方法，在阐释农村社会资本多维特性并构建社会资本异质性测度指标的基础上，探讨欠发达地区农村社区老年健康促进机制。构建分层线性模型考察农村社会资本异质性对老年健康的影响效应；构建混合Logit模型考察健康不平等和社会资本异质性对农村社区老年健康促进的影响机理；构建Bass扩展模型阐释健康促进在农村社区老年人群中的扩散规律，并揭示农村社区中群体"意见领袖"的形成机制。以此为基础，提出完善我国农村社区老年健康促进的政策建议。

.贫困和疾病是所有社会问题的核心，健康促进对于提高老年人的健康水平具有重要作用。项目以健康不平等和社会资本异质性分析中的科学问题为切入点，借助田野调查、实验研究和回归分析技术等核心方法，阐述了健康不平等和社会资本异质性对欠发达地区农村社区老年健康促进的影响机理，揭示了健康促进在农村社区老年人群中的扩散规律，分析了农村社区中群体“意见领袖”的形成机制等关键问题。研究发现，社会资本禀赋不但对欠发达地区老年人口的健康水平具有显著影响，而且对于老年人口的健康促进活动的参与意愿具有积极影响。同时，研究识别了三种类型的意见领袖，即文化型、经济型、组织型。研究结果对于改善农村社会组织和社区管理水平具有科学意义。项目进展顺利，基本达到预期研究目标。
