Clinical research demonstated the predominance(preponderance) of acupuncture on modulation biphsic states of anxiety and depressive which make up the deficient of depressant treatment. Empirical study indicate the relationship between stressed animal behavior and stress time course: anxiety within stressed while depressed within post-stressed. Central nitric oxide alteration synchronized with model's behavior: increased within stressed period while decreased within post-stressed. Hypothesis was raised that electro-acupuncture modulate biphasic states of depression was mediated by NO system. The investigation will observe alteration of NO system and model's behavior treated by electro-acupuncture respectively and its relationship. The model will be duplicated by chronic unpredicted mild stress(CUMS) and drugs. CUMS model will be investigated by stages (stressed, post-stressed) according to its feature of behavior alteration. NOS inhibitor will be used to resist the anti-depression effects by acupuncture. Thus counterevidences would be presented that NO is critical to acupuncture anti-depression.
临床研究显示针刺抗抑郁具有调节焦虑与抑郁双重状态的优势,有效地弥补药物治疗的不足。实验研究显示应激模型动物行为变化与应激时程相关:应激期以焦虑行为为主,后应激期表现为抑郁行为。中枢一氧化氮(NO)系统变化与模型动物行为变化同步:应激期浓度增加,后应激期下降。这一特点与电针调节抑郁症双重状态相吻合。故而建立了电针抗抑郁调节焦虑与抑郁双重状态是通过NO系统介导的工作假说,观察电针对慢性不可预见温和应激(CUMS)和药物动物模型焦虑与抑郁双重状态的调节作用,以及与NO系统变化的相关性。其中CUMS模型将根据应激时程的行为变化特点采取分期研究(应激期、后应激期)。本研究还将利用药物阻断的方法反证NO是针灸抗抑郁的关键环节。
“NO系统介导电针调节抑郁症双重状态的研究”的主要研究目标在于探讨NO系统在电针抗应激调制焦虑与抑郁双重状态的作用及其机制。本项目以足底电击和慢性不可预见性温和应激(chronic unpredictable mild stress,CUMS)制备应激动物模型,采用电针和药物干预的方法研究NO系统在电针对应激反应中焦虑与抑郁双重状态的影响。结果:①电针具有治疗应激诱发的双相性焦虑与抑郁作用:电针对电击足底应激模型大鼠抗焦虑作用,电针对CUMS模型大鼠的抗焦虑与抗抑郁作用,电针对CUMS诱发的焦虑与抑郁共病模型的抗焦虑和抗抑郁双相作用。②焦虑是应激的反应性行为表现,短期急性应激致焦虑样行为学改变;抑郁是应激的结果性行为表现,长时程应激导致模型大鼠抑郁行为。应激导致海马NO系统变化规律为:应激引发海马iNOS表达升高;eNOS表达降低;由于海马神经元损伤,nNOS既可表现出低表达,也可表现出代偿性表达增高。③电针调节NO系统、抗焦虑与抗抑郁的规律为:电针降低应激增高的iNOS表达,升高应激降低的eNOS表达,双向性调节nNOS表达:模型大鼠海马nNOS下降时,电针增加其表达;模型大鼠海马nNOS表达上升时,电针降低其表达。④电针抗抑郁与改善模型大鼠海马神经元结构损伤、降低nNOS代偿性表达上升有关。⑤电针与NOS拮抗剂表现出协同抗抑郁、抗焦虑效应。⑥鉴于电针增加eNOS表达的效力有限,对比了放血疗法与电针的对应激模型大鼠抗抑郁的效应差别,发现了放血疗法对eNOS的调节效应强于电针,同时也具有降低iNOS的作用。放血可以降低应激后焦虑模型大鼠海马的炎症因子水平。.科学意义与应用前景:本项目明确了电针对于NO系统的调节作用及其效应方向,研究结果可以为以下临床应用提供应用理论基础:针灸治疗应激性心身疾病,针药结合治疗抑郁症;扩展的对于放血疗法的研究结果为放血疗法治疗抑郁症的临床应用提供一定的理论线索。
