Diabetic nephropathy (DN) remains the leading cause of end stage renal disease (EASD) worldwide. It has been shown previously that the susceptibility to type 2 DN is associated with the Carnosine dipeptidase 1 gene (CNDP1) which locates on chromosome 18q22.3. Type 2 diabetic patients with homozygous for the CNDP1 (CTG)5 repeat have a reduced risk to develop DN as compared to all other genotypes, e.g. (CTG)6, (CTG)7 and (CTG)8.CNDP1 encodes for the serum carnosinase 1 (CN-1). CN-1 degrades carnosine specifically. .Carnosine is previously proved to prevent oxidation, glycation and apoptosis etc.. Also, carnosine could scavenge the methylglyoxal(MGO),and therefore prevent the development of DN. However, the mechanism is still unclear. Carnosine could be rapidly degraded by serum CN-1, which limits its clinical use. .Hereof, we intend to 1) recruit T2DM patients with or without DN, and delineate the relevance of serum and urinary CN-1 to MGO. 2) delineate the proteomic correlation between CN1-carnosine system and MGO in vitro by making use of isobaric tags for relative and absolute quantification(iTRAQ)..This new project would hopefully warranted a new biomarker or target for DN.
2型糖尿病肾病的发展与染色体18q22.3上CNDP1基因信号肽编码区的CTG重复序列的多态性相关。纯合CNDP1 (CTG)5是糖尿病肾病的保护因素。CNDP1编码的蛋白为血清肌肽酶-1(CN-1),是肌肽的特异性水解酶。.肌肽有多种保护作用,包括抗氧化、抗糖基化、抗凋亡等。既往研究表明肌肽可通过减少甲基乙二醇(MGO)的生成而阻止糖尿病肾病的发展,但具体机制尚未明确。机体内CN-1对肌肽的快速水解作用限制了肌肽的保护作用,这是肌肽广泛应用的难点所在。.因此,本研究拟:1)在伴或不伴有肾病的2型糖尿病患者血尿CN-1水平和MGO,了解糖尿病肾病患者的血清MGO是否随肾病的发展及CN-1的水平升高而增高;2)在肾近曲小管上皮细胞(PTEC)中,探索肌肽是否能够减轻MGO造成的细胞损伤及蛋白组学改变。在患者中再次明确信号通路内关键蛋白,为糖尿病肾病治疗的新靶点提供实验证据。
2型糖尿病肾病的发展与编码CN-1的CNDP1基因相关,肌肽能够被CN-1特异性水解,既往研究表明肌肽可通过减少甲基乙二醛(MGO)的生成而延缓DN的发展,前期研究证实MGO具有细胞损伤作用以及肌肽能够保护细胞,但具体机制未明。本项目通过招募伴或不伴有肾病的2型糖尿病患者测量血尿CN-1浓度和MGO、体内实验(STZ+手术创面造模)相结合的方法,发现了血尿CN-1与肾功能和炎症具有相关性,为目前2型DN的临床诊断、治疗及预后提供初步的参考证据,另外在体外实验(MGO/肌肽作用于HK-2细胞)中发现不同条件下细胞的蛋白组学发生了显著的改变,并通过WB初步验证了相关的蛋白表达,为发现DN防治的新靶点提供了实验证据。
